A primeira temporada, inicialmente, deveria ter tido treze episódios, quantidade pedida para o show antes de ele estrear no midseason. Contudo, a ABC decidiu encerrar a temporada no nono episódio, ao ver que ele constituía um final de temporada mais dramático e chamativo, com George esmurrando Alex ao descobrir que ele fora, indiretamente, responsável por ele ter contraído sífilis, e Meredith descobrindo que Derek era casado quando sua esposa, Addison, chega ao hospital. Assim, os 4 últimos episódios deveriam ser realocados para a 2.ª temporada, se tornando os quatro primeiros episódios desta. Como a ABC já havia encomendado 22 episódios para a segunda temporada, esta acabou ficando bastante extensa, com 26 episódios. Mais tarde, um 27.º episódio acabou sendo adicionado aos planos. Tal contagem não inclui os capítulos especiais de recapitulação “Straight to the Heart” e “Under Pressure”.
A segunda temporada foi para o ar pela primeira vez em 25 de Setembro de 2005 e chegou ao fim nos dias 14 e 15 de Maio de 2006, com um fim de temporada de cerca de três horas que cobriu as duas noites.
